Family carers of individual’s living with Huntington’s Disease (HD) manage a distinct and unique series of difficulties arising from the complex nature of HD. This paper presents the validation of the definitive measure of quality of life for this group. The Huntington’s Disease Quality of Life Battery for carers (HDQoL-C) was expanded and then administered to an international sample of 1716 partners and family carers from 13 countries. In terms of the psychometric properties of the tool, exploratory analysis of half (...) of the sample, demonstrated good internal consistency and reliability. Some items on the full version did not meet psychometric thresholds and a short version (HDQoL-Cs) was developed based on more stringent criteria. Confirmatory Factor Analysis of the model structure showed a good fit for all Factors and, indicated that the HDQoL-C and HDQoL-Cs are psychometrically robust measures of quality of life. We found that carers who lived with and looked after their spouse/partner had reduced sense of coping, hope for the future and overall quality of life. Carers with children who were at risk, carried the gene or were symptomatic also had poorer quality of life outcomes. Findings indicated the HDQoL-C and HDQoL-Cs are valid in multiple languages and across varied cultures as measures of self-reported quality of life in family carers of individual’s living with HD. These psychometrically validated tools can aid and guide the implementation of therapeutic interventions to improve life quality in this population. (shrink)

The clinical introduction of medical devices often occurs with relatively little oversight, regulation and (long-term) follow-up. Some recent controversies underscore the weaknesses of the current regime, such as the complications surrounding the metal-on-metal hip implants and the scandal surrounding the global breast implant scare of silicone implants made by France's Poly Implant Prothese (PIP) Company. The absence of national registries hampered the collection of reliable information on the risks and harms of the PIP breast implants. To warrant long-term safety, a (...) case can be made for mandatory post-marketing surveillance by means of the establishment of compulsory registries. In this edition of Public Health Ethics, Schofield calls for debate on how such a registry system should be initiated and maintained and how it would relate to the ethical requirement of consent. Here we use breast implant registries as a case to discuss whether and when a so-called ‘thick opt-out’ would be an appropriate method to include people in medical device registries. We conclude that a thick opt-out procedure for medical device registries is only justifiable in cases where inclusion does not involve burdens (or very low), when it does not involve a sensitive subject and when the data are stored anonymously (or at least not directly linked to the medical record). Otherwise, inclusion should be sought by means of an opt-in. (shrink)

Emotional stimuli (e.g. words, images) are often remembered better than neutral stimuli. However, little is known about how memory is affected by an environmentally induced emotional state (without any overtly emotional occurrences) – the focus of this study. Participants were randomly assigned to discovery (n = 305) and replication (n = 306) subsamples and viewed a desktop virtual environment before rating their emotions and completing objective (i.e. item, temporal-order, duration) and subjective (e.g. vividness, sensory detail, coherence) memory measures. In both (...) samples, a Partial Least Squares Correlation analysis showed that an emotional state characterised by high negative emotion (i.e. threat, fear, anxiety) and arousal was reliably associated with better memory in both objective (i.e. item) and subjective (i.e. vividness and sensory detail) domains. No reliable associations were observed for any temporal memory measures (objective or subjective). Thus, an environmentally induced state of negative emotion corresponds with enhanced memory for indices of episodic memory pertaining to “what” happened, but not necessarily “when” it happened. Professor Lewis and I have some important differences of opinion regarding the identity and distinctness of conscious persons, which it will be well to try to clarify on the present occasion, first of all by enumerating a number of points on which we are, I think, in agreement. Both of us believe in the existence of individual persons, each of whom can be said to live in a ‘world’ of his own intentional objectivity, a world ‘as it is for him’, (...) which differs in a considerable extent, both in content and emphasis, from the world as it is for anyone else. Both of us further believe that all these intentionally objective worlds for a large part coincide in content, and are in fact excerpted from a more comprehensive real world which is common to us all, and which, in addition to in some sense including all such intentionally objective worlds, also includes many real material objects which exist regardless of our intentionality, and which further includes our own material bodies, which appear in so central a manner in each of our intentionally objective worlds. Both of us believe in matter as a transcendent reality, as well as an intentional object, and are content to accept the dicta of science as to the most probable view of its structure. We are in fact quite Cartesian and Lockean in our belief in the primary and secondary qualities of matter. We believe further that our intentional subjectivity is geared causally into our material objectivity, and that the gearing takes place, in some inscrutable manner, in our nervous systems. We both also believe that our intentional subjectivity transcends bodily mechanisms and instrumentalities, and can be liberated from the latter, but that, when thus liberated, our subjectivity may still affect some sort of an intentionally objective material body such as we wear in dreams, a body in which it will manifest itself to itself and to others much as we do in our dreams and fantasies. Cloth, $65.00.This collection has the commendable aim of challenging the view that in Plato’s “late” works the dialogue form is a mere formality adding little to the argumentative content, a view (...) still widespread among “analytic” scholars. The qualification is necessary because this view has already been seriously challenged by “nonanalytic” interpreters. It is therefore surprising that the editors chose contributors who almost all “share the analytic’ approach which is prominent in Anglo-American scholarship on Plato,” despite their admission that this approach has focused more on arguments than on the dialogue form (2, 299–300). Why seek to reinvent the wheel without the help of those who have already invented it? As a result, while the volume certainly makes an important contribution to our understanding of the form of this group of dialogues, it is much more successful in providing helpful and illuminating readings of their argumentative content: the detailed and rigorous analyses of McCabe, Malcolm Schofield (both on the Parmenides), and Gail Fine (on the Theaetetus) deserve special mention. The aim of the present review, which could not even begin to do justice to what the volume provides in the latter area, is therefore limited to documenting its mixed success in addressing some fundamental questions about form that have already occupied nonanalytic scholars (perhaps too exclusively):1) Does the main speaker speak for Plato?Given the great variety of main speakers in the “late” dialogues (Socrates, Parmenides, the Eleatic Stranger, Timaeus, the Athenian Stranger), each with a distinct personality and a distinct method, no attempt to understand the form of these dialogues should avoid this question. Gill raises it (293–95) and Schofield provides the most explicit response in denying that any interlocutor in a Platonic dialogue “can be assumed to be merely a spokesman for Plato’s own views” (51). More importantly, the essays by Dorothea Frede (on the Philebus) and Catherine Osborne (on the Timaeus), probably the most valuable in the volume, show what results can be achieved without this assumption. On the other hand, Michael Frede (on the Sophist) and Christopher Rowe (on the Politicus; see 158 and cf. 172 n.48) simply assume without argument that the Eleatic Stranger = Plato. All other contributors make the “spokesman” assumption, either implicitly or explicitly (see Christopher Bobonich’s reading of the Laws, 254–55).2) What is the significance of the dramatic context, especially dramatic chronology and characterization?As often noted, the Parmenides is set in Socrates’ youth and is in this sense the earliest [End Page 311] dialogue, while the series Theaetetus-Sophist-Politicus is set immediately before Socrates’ trial and condemnation. McCabe and Schofield are silent about the first fact. M. Frede briefly discusses the second (146–47), but adds little to what has already been said by others. As for the characterization of specific interlocutors, this volume overall has surprisingly little to offer. The essays of D. Frede and Osborne, however, are exemplary in this area.3) Why did Plato choose to write these works as dialogues rather than as treatises?Bobonich, D. Frede, and Osborne succeed in demonstrating the significance and even necessity of the dialogue form, an extraordinary achievement given the dialogues they discuss. Osborne in particular brilliantly shows how the dialogue form undercuts the common tendency to read the Timaeus as a treatise on natural science. Other contributors are less successful in answering this question central to the volume’s task. Schofield takes himself to be reading the Parmenides as a dialogue (51) because he, unlike Vlastos, interprets the Third Man Argument (second version) in the wider context of other arguments in the dialogue, including those of the second half. But “reading arguments in context” is demanded by any philosophical text and has nothing per se to do with the dialogue form. Fine reads the first part of the Theaetetus as the dialectical elaboration and refutation of an opponent’s view (109), but this could just as well be... (shrink)

All recent memory theories of hippocampal function have incorporated the idea that the hippocampus is required to process items only of some qualitatively specifiahle kind, and is not required to process items of some complementary set. In contrast, it is now proposed that the hippocampus is needed to process stimuli of all kinds, but only when there is a need to associate those stimuli with other events that are temporally discontiguous. In order to form or use temporally discontiguous associations, it (...) is essential to maintain some memory of the first component until the second component has occurred. When the temporal gap to he spanned is small, and the number of items to be temporarily retained is low, a limited-capacity, short-term store is sufficient to allow associations to be formed. Such a store is presumed to operate in parallel with the hippocampus in normal animals. Hippocampal damage disrupts a much higher-capacity store that has a slower decay rate, and so leaves animals with only a very limited ability to form temporally discontiguous associations. Hippocampal damage, however, is not held to affect the long-term storage of associations of any kind, if they can be formed. Analyses of both new and existing data are presented to show that by classifying tasks in terms of the need to use a temporary memory store to retain temporally discontiguous information one can cut right across existing classifications as well as achieve a better fit to the data. The hippocampus thus seems best described as a high-capacity, intermediate-term memory store. (shrink)

This is a very valuable study of the relations, as regards affinity and mutual influence, of two major philosophers who are now more and more being assessed at what we may hold to be their immense true worth. Both were philosophers who brought a form of Platonic realism, quite out of fashion at the time, into their interpretation of logical and mathematical concepts and principles, and who moved away from the psychologistic approaches which see such concepts and principles merely as (...) a set of forms and rules which govern our actual human thinking and its linguistic expression, and whose normative or standard-fixing aspects have their roots in the mere way in which our minds work and the mere ways in which the actual world fits in with their workings. But both thinkers, at a higher level, moved on to a view in which something like a transcendental Kantian Reason served as the ultimate foundation both of the forms and guiding principles of referential thought and of the factual empirical world whose structure this thought tries to encompass and to articulate. Both philosophers further seek in the mediating concept of Sinn or sense, as closely connected with the uses of language as with the objective matters with which language deals, the link between thought and other subjective orientations, on the one hand, and the various objective matters with which thought and language are concerned. Both thinkers, however, display great obscurities at certain points in the working out of their thoughts, by which obscurities their relation to one another is also at certain points rendered obscure. The problems of internationalism often look different and require different solutions depending on one’s particular cultural standpoint. Contemporary Indian philosophers find that their philosophical efforts have long been too much oriented to the West, and that what Indian philosophy now needs is less international influence and more rootedness in their own rich tradition. This is needed for a more balanced international dialogue. The goal of internationalism in philosophy is not to bring into being a world‐philosophy, but a global conversation on (...) philosophical problems that is not inhibited by differing approaches. The aim of this paper is to give a characterisation of religion and the Religious Spirit, basing itself on the Platonic assumption that there are Forms, salient jewels of simplicity and affinity, to be dug out from the soil of vague experience and cut clear from the confusedly shifting patterns of usage, which will give us conceptual mastery over the changeable detail in a given sector. It will further be Platonic in that it will not seek to discount the deep (...) gulfs between the species into which religion qua genus divides itself, i.e. its theistic, polytheistic and atheistic subvarieties, taking it to be of the essence of a true genus to extend itself over mutually exclusive species, only being what it is by including in its sense the alternatives which are thus mutually exclusive. And my treatment will be Platonic, thirdly, in that it will endeavour to delimit the Religious Spirit by, on the one hand, setting it over against what it excludes, all purely this-world talk and life which is quite irreligious, and by, on the other hand, opposing it to forms of talk and life which fall short of it in various ways or which deviate from it variously, thereby likewise contributing to our understanding of what it is. The practice of Plato, which could study the deviations from his ideal city in order to confirm his notion of its structure and excellence, and which also paired every ideal pattern with its opposite— piety with impiety, justice with self-interested tyranny, etc.—is plainly one to be followed: Plato, as we know from a citation from his contemporary Hermodorus in one of the Aristotelian commentators, always set beside the ‘in itself’ of the pure Form the deviant and the wholly negative which were nonetheless part of its sense. Religion will therefore stand before us as a target that it is possible to fall short of or miss altogether as well as to hit squarely, and we shall try by a series of glancing darts to end by hitting it squarely. In his award-winning book _The Philosophy of Edmund Husserl: A Historical Development_, J. N. Mohanty charted Husserl's philosophical development from the young man's earliest studies—informed by his work as a mathematician—to the publication of his _Ideas_ in 1913. In this welcome new volume, the author takes up the final decades of Husserl's life, addressing the work of his Freiburg period, from 1916 until his death in 1938. Indian philosophical thought has been deeply metaphysical, and it is no surprise that, faced with the anti-metaphysical thrust of contemporary philosophy, one of the issues uppermost in the minds of Indian thinkers is the question of the possibility of metaphysics. In recent philosophical literature, two tendencies are discernible: an attempt to defend metaphysics in the traditional grand style, and a concern with the idea of descriptive metaphysics as an alternative. For the former, we may turn to Kalidas Bhattacharyya and J. (...) N. Chubb; for the latter to J. N. Mohanty and S. N. Ganguly. (shrink)

It would be a mistake to attempt to identify in modern terms the disease of Galerius described so graphically by Lactantius, Mort. 33. Consumption by lice or worms, if not genital ‘gangrene’, was a typical end for a tyrant or the impious, and there must be an element of literary exaggeration in Lactantius' account. But whatever one makes of the nature of the illness, Lactantius did set out to give the passage a scientific plausibility by his use of technical medical (...) phraseology, and by an allusion to a medical theory at 33.7. Recognition of this theory allows one to settle the text at one point, where editors have failed to agree. There is also a second place in the chapter where familiarity with medical Latin points one towards the solution of a textual problem. (shrink)
